Lippincott Q&A Review for NCLEX-RN
Rationale-focused explanations — critical for understanding why an answer is correct, not just memorising.
🎯Prometric Exam Strategy
💡

Read the stem fully: MOH questions often include distractors. Identify the subject (patient / nurse action), the problem, and what is being asked before reading options.

🔢

Elimination technique: Rule out obviously wrong answers first. If two options look similar, one is usually correct — focus on the clinical distinction.

⏰️

Time management: With ~150 questions in 3 hours, allocate about 70–80 seconds per question. Flag difficult questions and return after completing all others.

🌠️

Priority questions: When asked “what do you do first?” — apply ABCs (Airway, Breathing, Circulation), then safety, then comfort.

📋

Pharmacology calculations: Expect 5–10 calculation questions. Practise IV drip rates, dosage calculations and unit conversions until you are fast and accurate.

📄

Exam day logistics: Arrive 30 min early with valid photo ID. You will be photographed and fingerprinted. Scratch paper and pencil are provided by Prometric — no personal items in the room.

Practice Quiz — 15 MOH-Style MCQs
Question 1 of 15
A nurse is preparing to administer medications. Which action best demonstrates adherence to the five rights of medication administration?
Using two patient identifiers (e.g., name + date of birth or ID band) verifies the right patient — a core component of safe medication practice.
Question 2 of 15
A patient on IV heparin therapy suddenly develops shortness of breath, tachycardia and hypotension. What is the nurse's PRIORITY action?
These signs suggest heparin-induced bleeding or pulmonary embolism. The immediate priority is to stop the causative infusion and notify the physician to obtain further orders (including potential protamine sulphate).
Question 3 of 15
When performing hand hygiene with alcohol-based hand rub, how long should the nurse rub their hands together?
WHO guidelines specify rubbing alcohol-based hand rub for 20–30 seconds (until hands are dry). This ensures adequate antimicrobial contact time.
Question 4 of 15
A physician orders morphine 4 mg IV. The available vial contains morphine 10 mg/mL. How many mL should the nurse administer?
Using the formula: Volume = Dose ÷ Concentration = 4 mg ÷ 10 mg/mL = 0.4 mL.
Question 5 of 15
A nurse is caring for a patient in contact precautions for MRSA. Before leaving the room, the nurse should remove PPE in which order?
CDC sequence: Gloves first (most contaminated) → hand hygiene → Gown → Mask/goggles → final hand hygiene. This prevents self-contamination.
Question 6 of 15
A nurse is assessing a patient's pressure injury. The wound has full-thickness skin loss with visible subcutaneous fat but bone, tendon and muscle are NOT exposed. This is classified as:
Stage 3 = full-thickness tissue loss with visible subcutaneous fat. Stage 4 involves exposed bone, tendon or muscle.
Question 7 of 15
A nurse notes that a patient's IV infusion rate is ordered at 125 mL/hour using a macro-drip set (10 drops/mL). What is the correct drip rate in drops per minute?
Formula: (mL/hr × drop factor) ÷ 60 = (125 × 10) ÷ 60 = 1250 ÷ 60 ≈ 20.8 ≈ 21 drops/min.
Question 8 of 15
A postoperative patient reports pain of 8/10 and requests pain medication. The nurse reviews the chart and finds no current analgesic order. What is the FIRST action?
No medication may be administered without a valid physician order. The nurse must contact the physician and advocate for the patient to obtain an appropriate analgesic order.
Question 9 of 15
According to Maslow's hierarchy of needs, which patient need should a nurse address FIRST?
Physiological needs (airway/oxygenation) are at the base of Maslow's hierarchy. SpO2 of 88% represents a physiological emergency that takes priority over safety, love/belonging and esteem needs.
Question 10 of 15
A nurse is teaching a new mother how to care for her newborn's umbilical cord stump. Which instruction is CORRECT?
Current evidence supports dry cord care — keeping the stump dry, clean and exposed to air. Folding the nappy below the cord prevents moisture. Submerging in water is contraindicated until the cord falls off.
Question 11 of 15
A patient with a nasogastric (NG) tube is being fed. Before commencing the feed, what is the MOST important nursing action?
Verifying NG tube placement before feeding is the most critical safety step. Aspiration of gastric contents with pH ≤5.5 or X-ray confirmation are accepted methods. Administering feed into a misplaced tube can cause aspiration pneumonia.
Question 12 of 15
A nurse is caring for a patient taking warfarin. Which laboratory value requires IMMEDIATE physician notification?
The therapeutic INR range for most indications is 2.0–3.0. An INR of 5.8 represents supratherapeutic anticoagulation with high risk of bleeding and requires immediate medical attention and likely withholding of warfarin.
Question 13 of 15
A nurse witnesses a colleague administer a wrong medication dose to a patient. After ensuring patient safety, what is the nurse's ethical obligation?
Nurses have a professional and ethical obligation to report medication errors through established incident reporting systems. This promotes patient safety and systemic learning. Patient safety always takes priority over protecting a colleague.
Question 14 of 15
A nurse is assessing a patient for risk of deep vein thrombosis (DVT) post-surgery. Which finding is MOST indicative of DVT in the lower limb?
Classic DVT signs: unilateral swelling, warmth, redness, and a positive Homans' sign (pain on dorsiflexion). Bilateral symptoms suggest systemic causes rather than DVT. Pallor and absent pulses suggest arterial insufficiency, not venous thrombosis.
Question 15 of 15
A community health nurse is planning a health education session on type 2 diabetes prevention. Which teaching approach is MOST effective for adult learners?
Adult learning principles (andragogy) emphasise self-direction, relevance to real life, and active participation. Interactive methods with return demonstration are more effective than passive lecture or written assignments for behaviour change in health education.

⚠️Licence Cancellation & Transfer
📄

Leaving UAE: Arrange licence cancellation through MOHAP portal before your visa is cancelled. Uncancelled licences can create complications for future UAE applications.

🕒

NOC from Employer: If changing employers within the Northern Emirates, obtain a No Objection Certificate (NOC) from your current employer. Your new employer will sponsor a licence transfer.

📩

Re-activation: If your MOH licence lapses, you may re-apply but will likely need to repeat the DataFlow verification and potentially re-sit the exam depending on the gap period.

🔄Licence Portability & Multiple Licences
ScenarioCan you work?Action Required
Hold MOH → Work in SharjahYESNone — MOH licence covers Sharjah
Hold MOH → Work in RAK / Ajman / UAQ / FujairahYESNone — MOH licence covers all Northern Emirates
Hold MOH → Work in DubaiNOMust pass DHA exam and obtain DHA licence

Hold MOH + DOH simultaneouslyYESLegal — maintain both licences and CME separately
💡

Strategy Tip: Many nurses in Sharjah commute to Dubai. If you regularly work shifts in both emirates, obtaining both MOH and DHA licences gives you maximum flexibility. The DHA exam is generally harder — take MOH first, gain experience, then attempt DHA.

🏫CME (Continuing Medical Education) Guidance
  • ● Accredited workshops and conferences
  • ● Online learning modules (MOHAP-approved providers)
  • ● BLS / ACLS / PALS recertification
  • ● Journal club and peer-reviewed publications
  • ● Hospital in-service training (with MOHAP approval)

Track all CME activities in the MOHAP portal as you complete them — do not leave recording to the end of the cycle. Some activities require pre-approval.

⏰️

Aim for 15 CME hours in year 1 and 15 in year 2 to spread the workload evenly across your licence cycle.

Dress code: Conservative dress is expected in public areas in Sharjah and Ajman — shoulders and knees covered. Within hospital facilities, your uniform is sufficient. RAK and Fujairah are slightly more relaxed.

🌟

Ramadan: Eating, drinking and smoking in public during daylight hours is prohibited — this applies to all residents regardless of religion. Hospital schedules adapt with adjusted shift patterns.

🗣️

Language: Arabic is the official language but English is the working language across all Northern Emirates hospitals. Basic Arabic phrases are appreciated by patients and local colleagues.

💪

Community: Large Filipino, Indian, and other expat nursing communities are well-established in Sharjah and RAK. Peer support networks and professional WhatsApp groups make settling in much easier.
